By signing and submitting this Claim Form, I certify that:

  1. between May 10, 2019, and November 14, 2025, I made one or more purchases at a Michael Kors Outlet store, in part because they were advertised at discounted prices, and I have not received a refund or credit for my purchase(s);

  2. I have received Notice of the Class Settlement in this Action and that I am a member of the Settlement Class of persons described in the Notice;

  3. I agree to release all the claims, known and unknown, stated in Section 2.8 of the Settlement Agreement;

  4. I submit to the jurisdiction of the Superior Court of California, County of San Diego, with regard to my Claim and for purposes of enforcing the release of claims stated in the Settlement Agreement;

  5. I am aware that I can obtain a copy of the Full Notice and Settlement Agreement on this Settlement website page or by writing the Claims Administrator at the email address: info@MichaelKorsOutletSettlement2026.com or the postal address: McCall et al., v. Michael Kors (USA), Inc., Case No. 25CU041352N, c/o Simpluris, P.O. Box 25226, Santa Ana, CA 92799; and

  6. under the penalty of perjury of the laws of the State of California that the foregoing is true and correct to the best of my knowledge and that providing my signature below constitutes electronic signature on the date of its submission.
